The
Main Street Project Transition Services Team provide counselling,
support, and advocacy to clients throughout our facility, focusing
on areas such as mental health, housing, client assessment, case
planning, and community outreach.
Their
goal is to "work with motivated clients through a process
of change that results in a healthier and more independant lifestyle."

Team members
assess and assist clients with the establishment of plans to
address such issues as:
- substance
abuse;
- housing;
- employment;
- health;
- money management;
- relationship;
- counseling;
- legal issues;
- personal
development;
Through one
on one counseling sessions or referrals to appropriate outside
agencies, the team works with individuals to help them attain
their goals. The
team also facilitates a variety of educational and development
sessions with clients utilizing the Main Street Project's Detoxification
Unit, and Mainstay Residence.
