Transition Services

The Main Street Project Transition Services Team provide counseling, support, and advocacy to clients throughout our facility, focusing on areas such as mental health, housing, client assessment, case planning, and community outreach.  Team members also offer follow-up services to clients who move back to their communities.

The goal of Transition Services is to "work with motivated clients through a process of change which results in a healthier and more independent lifestyle."

Team members assess and assist clients with the establishment of plans to address such issues as:

  • substance abuse;
  • housing;
  • employment;
  • health;
  • money management;
  • relationship;
  • counseling;
  • legal issues;
  • personal development;

Through one-on-one counseling sessions or referrals to appropriate outside agencies, the team works with individuals to help them attain their goals. The team also facilitates a variety of educational and developmental sessions with clients using the Main Street Project's Detoxification Unit and Mainstay Residence.
